You can use this CSS code to set max width to post images. Insert this CSS code in Dashboard > Forums > Settings > Styles > Custom CSS Code textarea, save it, delete all caches, go to forum front-end and press Ctrl+F5:

I however see some posts on this forum that have an image attachment but no image shows (see "capture.png"), have you not implemented the code on your forum?

Also I have installed the "Responsive Lightbox & Gallery" plugin which is set to show all images on my site in a lightbox however when I click on images on my wpforo forum it opens in a new window rather than a lightbox. Any idea what could be the issue?

I'm sorry but the answer is yes. After the Image embedding code and the  Advanced Attachment addon development there were no more request for this button. The file attachment button does the same and it's very simple to use, even if it's little bit not user friendly.
