First of all: Relax.
Then check the Page of the forum, does it have the Shortcode (check in TEXT mode) ?
If yes, then enable Wordpress DEBUG to a LOG and check for FATAL (serious) errors.
Because from what captures you posted, the forum is there (i mean in the db etc etc).

The line 95 in the code looks like this (see Attach):

I have changed the "WP..." to 'WP...'
