A fairly trivial point as I am able to enrol new users, but, when I receive the notification email the username and Email are blank - see attached. I'm presuming I should see such info, I see the username on password change emails.

Yes, same here.

Several months back, it used to give the info, but something must have changed - unsure if it was a deliberate change or not.

the issue with empty shortcode content would be fixed in the new version of the plugin 2.1.2
