What would be the best/easiest way to accomplish this:
I would like to create a set of groups with a badge/icon.
When someone creates and account they would be asked if they belong to any of the groups. If they do, then a badge/icon would be shown when they create a post.

I'm sorry, but you can't set a badge/icon to usergroup. You can manage Usergroup colors (Dashboard > Forums > Usergroup).

Please check out the wpForo – User Custom Fields addon. It allows you to create a custom Registration form with custom fields.
