my client wants to be able to register while adding a topic. Is it possible to do it without massive modifications to code?

I have no clue.

This requirs lots of custom development and template file customization.

I think the easiest thing you can do is to make the registration process one click.  That just means integrating a social login plug-in with WpForo and so if you have this, they can click to register with facebook, etc. and then they'll be set to post.  I did this on my site at http://ontheten.org/community  and it works beautifully using the AccessPress Social Login Lite plug-in.  Of course you can select a different plugin to accomplish this, but the one I used (by AccessPress) was very easy to configure.  It just gives the three main social media sites, facebook, twitter and google+.
