Remove wpForo from W3 Total Cache. No caching should be used in wpForo pages.

Could be a low memory issue. Try to find a way to check it. And if you have 128, double it or even more.

It seems that some update of wpForo or Wordpress did that to me also. You are the 3rd having this ... You will have to re-create/fix them manuall...
