Something very strange happened here, right after i logged out.
I saw a red popup at the top right, telling something about a wrong password provided for Paul D..... (The "Paul" i'm sure, the D....and the rest something faded quickly). I didn't had the time to fully catch it.
Apparenty someone was logging in? But why I saw that? Strange ....
Do you use the plugin "Simple History"? If yes, you can see the activity of who was logging in at the time that occurred. You should check it out if not for now, future issues.

Can you provide a screenshot when you face it again, We tested but couldn't make it happen.
Well popups fade out quickly, so probably i can't provide a screen shot.
