Today I have published a free companion plugin for wpForo to help admins moderate inappropriate profile images.

Warning - test this on your staging server first... and take a moment to read the readme

"When users upload profile images, we process them via Google Vision API Safe-search, which can recognise things like nudity. The results from the safe-search are shown a score against the user listing in WP Users admin. Periodically you can check if any scores are high and view the images yourself. This plugin does not block or otherwise interfere with the users uploads."

This is only available to download via github. https://github.com/Mistralis/wp-profile-moderation

As you might guess you need a Google Vision API key, which has a free quota of 1,000 images per month.
