We have some very active and old users whose posts now go into moderation every time they post something. We have played around with various spam settings but nothing seems to fix the problem.
Is there anything we should be doing to stop this?

Can you post ONE of those posts that go to moderation?
Also this is NOT working right with wpForo. NO CACHING. None. Exclude wpForo from caching, that could be the issue also with moderation:

The reply from user Antonio_F had to be moderated. He is a daily user of our forum and been a member for several months. All his posts need to be moderated/approved
I do not see ANY user reply from Antonio_F, all i see is "Anonymous".
And again, REMOVE Fastest Cache from wpForo.
