I've made it so guests can read but not participate. I think it was the default. When new content is posted, or things are edited, guests can't see it for days. It takes that long, and then suddenly it's available. Any ideas? Is this a cache issue? If so, why does it only affect guests?

It's a cache issue. If you ave a cache plugin, please read this instruction and exclude wpForo pages from your cache: https://wpforo.com/community/faq/wpforo-and-cache-plugins/#post-37150
