Hello friends, I hope to explain clearly what I want to do.
I have a forum of recommendations and what I want is that when someone clicks on the button to add a topic in a specific section of the forum, a template to fill out like this already appears:

I'm sorry but this is not an easy task to provide some solution here. You should find a developer for a custom job.

In addtion, I think this would be better to do with Topic and Posts editor custom fields. So you could create as much options and fields as you want. However at the moment we don't have such solution. This is already in our to do list but it's not planned in near future.
