wpForo 1.x.x [Closed] When I add a topic I will be directed to an error after adding topic then lose everything.

I was creating a topic, had the title, input information, three pictures and added a couple tags. 
 
When I clicked "Add Topic" I got the Screen in the attached picture. And all the data is erased and the topic is gone.
