When i post a new topic as a normal user i see in the logs that no notification is triggered after the topic is approved. However if i do it as moderator, and approval is not needed, then the notification goes to all the subscribed users
Any idea or similar observations?

The notification is triggered if there are some subscribed user to that topic or to that forum. There'll not be any notification if there are no subscribers.
In any case, if there is a real issue, this is something we cannot help to debug. wpForo doesn't have such problem for sure. This is a site specific issue, or even user/email specific issue. well i did a simple test and on a specific forum with new topic subscribers, notification came when moderator created a topic and didnt come when non-moderator created. it's kind of convincing me...
i can also give you access... how do we debug this?
