I get the message can't send confirmation email when subscribing to forums.
Emails where getting send on other things.
Any ideas?

Please make sure you you use the latest version and you don't have old forum template files in active WordPress theme.
Deactivate all plugin which have relation to antispam, security, email actions, registration and user system.
This is all done, the only registration plugin I have is your custom form plugin.

Maybe you could enable standard WP debug to a LOG and check for errors.
